On Friday, September 22nd, at approximately 2:30 pm, Sgt Carson with the Utah Highway Patrol requested that Trooper Lorensen respond to the scene of a traffic accident at the intersection of SR-40 and 1500 S.

When Trooper Lorensen arrived on the scene, he was told that one of the involved drivers was 20-year-old Mataya Angel. In the crash, she was alcohol restricted, and there was a strong odor of alcohol and open containers in her vehicle.

After Trooper Lorensen spoke with Sgt Carson, he attempted to conduct SFSTs on Angel. During these tests, Lorensen was only able to complete the HGN test, which showed a lack of smooth pursuit.

As Trooper was beginning to explain the other tests, Angel indicated that her injuries sustained during the crash were making it painful to complete the tests as instructed. Because Angel was unable to complete the other SFSTs, they moved on to conducting a PBT test, which indicated a positive result for alcohol.

Angel was transported to the Ashley Valley Regional Medical Center. Angel's injuries were checked at the hospital, and she submitted to a blood test. The blood test results were still pending when this affidavit was written.
