Sgt. Cannon received information about a tan Chevy Suburban that was involved in a hit-and-run accident, where the vehicle had struck a fence and fled the scene near 1300 West 500 South.
Radio traffic indicated the Suburban was last seen turning eastbound on 1500 South from 500 West.
Sgt. Cannon located the vehicle traveling eastbound on 1500 South, passing 500 East, and stopped the Suburban at approximately 675 East, 1400 South.
The vehicle came to a stop briefly, then sped off westbound through a residential area. The truck was intentionally swerving from one side of the road, almost crashing. The vehicle then turned right from 1375 South onto 570 East, which is a dead-end road.
The driver, 21-year-old Kaleb James Evans-Cook, was called out of the vehicle and secured in handcuffs behind his back. During this, Kaleb had a hard time following instructions.
Sgt. Cannon asked Kaleb if he had been consuming alcohol and he stated he had drank a little. He then said he hadn't drunk anything and stopped answering any further questions.
A records check on Kaleb revealed that his Utah driver's license was revoked for an alcohol offense. He is also required to have an ignition interlock device and is an alcohol-restricted driver. An ignition interlock device was not located in the vehicle.
An open container of Arnold Palmer alcohol was located in the center console of the truck.
Kaleb Evans-Cook was arrested for DUI (with two prior convictions within the last ten years), failing to stop at the command of a police officer, reckless driving, failure to install an ignition interlock device, alcohol-restricted driver, open container, and driving on a revoked license.
Kaleb was transported to Uintah Basin Health Care where Sgt. Cannon read him the DUI admonition. Kaleb refused and a search warrant for his blood was obtained. An ER nurse obtained blood samples from Kaleb. He was transported to the Uintah County Jail, where he was booked on the charges above.
Kaleb James Evans-Cook may post bail in the amount of $2,500.00.
